A Patient Asked Me if Anything Would Make His Prostate Smaller. After 19 Years in Urology, I Did Not Have an Answer.

Every option I am trained to offer manages the symptom or suppresses an enzyme. None of them was built to address the growth itself. This is what I found when I finally went looking, and why I am saying it in public.
DHT buildup in prostate tissue, mild to severe progression

You have bought the saw palmetto.

You have probably also bought the beta sitosterol, the pumpkin seed oil, and at least one bottle with a familiar face on the ad. You gave each one 30 days, maybe 60. You stood in your bathroom at 3 in the morning holding a bottle you had already stopped believing in.

I am a board-certified urologist. For 19 years I told men exactly like you that those bottles were a waste of money.

I was right about that. And it let me avoid a much harder thing to say out loud.

The two options I handed you instead were not aimed at the cause either.

One relaxes muscle so urine gets past the squeeze. One suppresses an enzyme and charges you somewhere else for it. Neither was designed to make an enlarged prostate smaller, and neither does. I just had a prescription pad, so nobody in that room questioned the gap.

It took a patient asking me a plain question for me to notice.

The Question I Could Not Answer

Arthur was 66. Machinist for 41 years, hands like a bag of walnuts, the kind of man who brings a folded index card with his questions written on it so he does not forget any.

He had been in my practice three years. I had him on tamsulosin and he was managing.

That morning he got to the bottom of his card, looked up, and said:

"Doc, I do not want to manage this. Is there anything that will actually make it smaller? Make it go back to what it was?"

I have been asked a lot of questions in that room. I had never been asked that one.

I gave him the honest answer, which is that the pill he was on relaxes the muscle around the urethra and does nothing to the gland, that finasteride can slow growth by suppressing DHT production and comes with the conversation he had already told me he was not interested in having, and that past those two there is surgery, which removes tissue rather than stopping the thing that grew it.

He listened to all of it and said: "So nobody is working on the actual problem."

Then he put his card back in his shirt pocket.

I thought about that drive home for a week.

What Is Actually Growing Your Prostate

Testosterone converts into dihydrotestosterone, DHT, which is substantially more potent at the androgen receptor. DHT binds receptors inside prostate tissue and signals those cells to multiply.

More cells means a larger gland. A larger gland wrapped around the tube urine travels through means a tighter squeeze, which is why the stream weakens, why you never feel finished, and why you are up at 2.

Bladder, prostate and urethra anatomy

Now here is the part the supplement aisle gets wrong, and it is not where most men assume.

Saw palmetto was aimed at DHT the whole time.

That is its stated mechanism. Weak inhibition of 5-alpha reductase, the enzyme that converts testosterone into DHT. You were not wrong about the target. You were sold a method of reaching it that the National Institutes of Health has now tested twice and found wanting.

What the NIH Found

The STEP trial (New England Journal of Medicine, 2006) compared saw palmetto with placebo at the standard dose and found no significant difference in symptoms or urinary flow.

The CAMUS trial (JAMA, 2011) tripled the dose to 960mg per day. Still no significant difference in symptom score, flow rate or prostate size.

Bent et al., NEJM · 2006  |  Barry et al., JAMA · 2011

Enzyme inhibition at supplement strength does not reduce DHT enough to change what the tissue does. That is the whole story of the shelf. Right target, wrong method, repeated across a dozen brands and a decade of your money.

And here is what nobody told Arthur, or you. Even perfect enzyme inhibition would only address DHT that has not been made yet. It does nothing about the DHT already in your bloodstream right now, binding receptors in your prostate this afternoon.

The Compound That Works on the DHT You Already Have

I went looking for anything that addressed circulating DHT rather than its production. That search took me somewhere I did not expect, which is a body of published work on a soy metabolite called equol.

Equol is not a herb and not an extract of one. Your gut bacteria make it from soy isoflavones, if you happen to have the right bacteria.

It binds DHT directly, with high affinity. A bound molecule cannot activate the androgen receptor in prostate tissue. It does not inhibit the enzyme and does not reduce how much DHT your body produces, which is the entire difference between it and finasteride.

Equol binding DHT before it reaches the prostate

Your testosterone is untouched. Your DHT still gets made and still does the other jobs it does. It simply cannot dock in the one place where docking was the problem.

Then I found the population data, and that is when I understood why this had never come up in 19 years of American practice.

59 Percent of Korean Men. 14 Percent of American Men.

A case-control study measured equol producer status across four countries in men without prostate cancer: 59 percent in Korea, 50 percent in Japan, 14 percent in the United States, 6 percent in Germany.

Age-stratified work in Korean men puts it higher in the bracket that matters most: 80 percent of men in their forties and 65 percent of men in their fifties were producers.

Everyone else is a non-producer. You can eat soy every day of your life and generate almost none of it, because the conversion step is bacterial, not dietary.

Akaza et al., Cancer Science · 2012  |  Song et al., Prostate · 2008

Roughly 6 American men in 7 cannot make the compound. It is not genetics and it is not discipline about diet. It is one bacterial conversion step that happens in their gut and not in yours.

The Second Pathway Nobody Mentions

Then I found the part that explains the men who go hard at DHT and still only get halfway.

Sex hormone binding globulin, SHBG, docks onto the membrane of prostate cells and starts a swelling cascade that runs independently of the DHT and androgen receptor route. Address one and leave the other running, and the tissue keeps expanding from the side you left open.

The lignans in standardized stinging nettle root bind SHBG and occupy those sites before the protein can dock. The most studied of them is 3,4-divanillyltetrahydrofuran. Nettle root has been in German clinical use for benign prostatic enlargement for decades.

1

Intercept the DHT already circulating.

S-Equol, the bioactive form, binds circulating DHT so it cannot activate the androgen receptor in prostate tissue. Production is untouched.

2

Block SHBG at the membrane.

Stinging nettle root lignans occupy SHBG binding sites on prostate cell membranes, closing the second route to tissue swelling.

3

Support the environment both work in.

Zinc, selenium, vitamin D3, vitamin E, green tea extract and the rest of the matrix. Supporting cast, not headline act.

Two pathways feeding one gland. Neither one requiring you to shut down an enzyme your body uses elsewhere.

I could not find a single product combining both at the doses the published work used. Then I found one that did.

What the Standard Path Costs

Supplement bottles on a bathroom shelf

Before the formula, it is worth seeing what the conventional arc actually costs a man over a decade. Rough US figures.

TAMSULOSIN (FLOMAX)

Monthly:$40 to $80 with insurance, considerably more without.
What it does:Relaxes smooth muscle around the urethra. Does not reduce gland size and does not affect DHT.
Known effects:Dizziness on standing, retrograde ejaculation, intraoperative floppy iris during cataract surgery.

FINASTERIDE

Monthly:$30 to $90.
What it does:Suppresses DHT production systemically. It genuinely works on the gland.
Known effects:Reduced libido and sexual side effects, reported to persist after stopping in a minority of users. This is the conversation that sends a large share of my patients back out the door.

THE SUPPLEMENT SHELF

Monthly:$25 to $60, usually across several brands before a man gives up.
What the research says:No better than placebo in two NIH trials, including at three times the standard dose.

PROCEDURES (TURP, REZUM, UROLIFT, GREENLIGHT)

Out of pocket:$8,000 to $25,000 depending on procedure and coverage.
Durability:Roughly 1 in 10 men need a repeat procedure within a decade.
Common tradeoff:Retrograde ejaculation, frequently permanent.

The money is not really the point, though it is ugly enough. The point is that a man can spend all of it and still be standing in a hallway at 3 in the morning, because nothing on that list was built to address what Arthur actually asked about.

What Is Actually in the Bottle

Saika Prostate Intercept

When I looked at the category, two things disqualified almost everything: trace doses of the named active, and proprietary blends that hide the amounts entirely.

Here is the formula that cleared both, with every amount printed.

Saika Prostate Intercept

S-Equol, 12mg per serving
The bioactive S-form. Binds circulating DHT so it cannot activate the androgen receptor in prostate tissue. The compound roughly 6 American men in 7 cannot produce in meaningful amounts.
Stinging Nettle Root Extract, 50mg per serving
Lignans that occupy SHBG binding sites on prostate cell membranes. The second pathway that DHT-only formulas leave running.
Green Tea Leaf Extract, 50mg
EGCG, included for its role in androgen signalling in prostate tissue.
Vitamin D3, 4mcg
Lower vitamin D status is commonly observed in men with benign prostatic enlargement.
Zinc, 2.75mg
The prostate concentrates more zinc than any other organ in the male body.
Selenium, 10mcg · Vitamin E, 3.7mg
Antioxidant pair, working on oxidative load in prostate and bladder tissue.
Vitamin C, 45mg · Vitamin B6, 0.4mg · Copper, 0.22mg
Connective tissue, hormone metabolism, and the zinc to copper balance that long-term zinc intake requires.

No proprietary blends. No water-diluted doses. Nothing in it that the NIH has already tested and found no better than placebo.

You Should Not Take My Word for This

Dr. Michael Hartwell
Dr. Michael Hartwell Board-Certified Urologist · 19 years

I am a urologist on the internet telling you that a supplement addresses something my prescriptions do not. You have every reason to close this page, and if you have been burned by six bottles already you should be harder on me than you were on them.

So do not believe me. Check it.

PubMed is free and public. Search equol and dihydrotestosterone and you will find the binding work. Search CAMUS saw palmetto and read what the NIH actually concluded, in their words, not mine. Search stinging nettle root and SHBG. None of that literature is mine, none of it is behind a paywall, and none of it needs me to interpret it for you.

Then look at the label and check that the doses on it match the doses in the papers. That is the test I applied, and it is the test that disqualified everything else I looked at.

What I will say plainly is the part I find hardest. I spent 19 years making it embarrassing for men to tell me what they were taking. Arthur asked me a reasonable question and the honest answer was that my entire toolkit was built around managing the consequence rather than the cause. That is not a conspiracy. Nobody hid anything from me. I simply never went looking, because the prescription pad made it unnecessary.

What Other Men Are Saying

Christopher M., 68 ✓ VERIFIED BUYER
★★★★★

I've seen just about every prostate supplement promise under the sun. What caught my attention with Saika was the different approach to ingredients and the simple two-capsule routine. No complicated steps, just something I can actually stick with.

Michael T., 64 ✓ VERIFIED BUYER
★★★★★

For years, I put my prostate health on the back burner. I finally started paying attention to what goes into these supplements, and Saika stood out to me. I appreciate knowing what I'm taking and why.

James R., 72 ✓ VERIFIED BUYER
★★★★★

I was skeptical because I've bought plenty of products that sounded too good to be true. Saika caught my eye because of its ingredient combination. I wanted a straightforward way to support my prostate health without adding another complicated routine to my day.

How It Compares

Tamsulosin: relieves the symptom by relaxing muscle. Does not reduce gland size.
Finasteride: reduces DHT production systemically, with the side effect profile that comes with that.
Saw palmetto: no better than placebo in STEP (NEJM 2006) and CAMUS (JAMA 2011).
Beta sitosterol and pumpkin seed oil: aimed at the same enzyme route, same ceiling.
Saika: two pathways rather than one, at the doses the research used, without suppressing an enzyme your body uses elsewhere.

60-Day Money-Back Guarantee

Take it for the full 60 days. If your flow, your nights and the way you feel have not meaningfully changed, email support@getsaika.com and you get every dollar back. No returning bottles, no forms, no retention call.

Arthur wanted a number, not a feeling. So do this the way he would.

Ask your doctor for an IPSS score before you start anything. It is 7 questions, it takes 4 minutes, and every urology office has the form sitting in a drawer.

Then take 2 capsules a day, 1 in the morning and 1 later on, for 8 to 12 weeks. Score yourself again on the same form.

You will have two measurements taken the same way, three months apart. They will tell you plainly whether this did anything for you, and you will not be relying on how you think you feel, on a testimonial, or on anything I have written on this page. If the numbers have not moved, the guarantee is still open.

How to Run It

  1. Get a baseline IPSS score before your first capsule.
  2. Choose your supply. Most men give it 12 weeks before judging it.
  3. Take 2 capsules daily, 1 morning and 1 later in the day.
  4. Keep taking it through the quiet stretch. The first weeks are usually uneventful.
  5. Rescore at 8 to 12 weeks on the same form.
  6. Decide from the two numbers. If they did not move, use the guarantee.

A note on timing

  • This works on a growth signal, not on muscle tone, so it does not produce an overnight change the way a muscle relaxant can.
  • Men who report a change generally describe it building over weeks rather than days.
  • Individual response varies, and some men will not respond. That is what the 60 days and the guarantee are for.

Two Options

OPTION 1

Keep Managing It

✗ Another bottle aimed at an enzyme route two NIH trials have already closed the door on.
✗ Another appointment where the only things on the table are a muscle relaxant and an enzyme suppressant.
✗ Another year of the gland doing what it has been doing, because nothing you are taking addresses the signal.
✗ The same question Arthur asked, still unanswered.
OPTION 2

Address the Signal and Measure It

✓ Baseline your IPSS score this month, before you start anything.
✓ Work on both pathways at the doses the research used, rather than one at a trace amount.
✓ Rescore at 8 to 12 weeks and decide from two numbers instead of a feeling.
✓ Keep it if the numbers moved. Get your money back if they did not.

Arthur was not asking for a miracle. He was asking whether anybody was working on the actual problem.

That question deserved a better answer than the one I gave him.

Saika Prostate Intercept
